Children must be 3 years old by December 1 and toilet trained to meet enrollment requirements.

Proof of physical exam and a record of up-to-date immunizations complete with physician’s signature must be on file at least two weeks prior to your child’s start date.

Enrollment Form complete with emergency numbers, Parent Questionnaire, Health History Form, Photo/Video Release Form, and Consent for Emergency Treatment Form must be submitted prior to your child’s start date.
